Course Enrollment Description
Complete the attached registration form and forward it with the appropriate tuition. Early registration is advised because enrollment is limited. A confirmation will be sent to you within 15 days via email upon receipt of your enrollment form and check. The registration fee includes course materials, continental breakfasts, and refreshment breaks.

This program provides contact hours for Occupational and Physical Therapists. Physical and Occupational therapists should inquire about therapy credits with their respective state boards.
Refunds and Course Cancellation
Requests for refunds must be received via fax or mail no later than 30 days before the course date. Beyond this date, no refunds will be issued. Refunds will be subject to a $50.00 processing fee.
Advanced Rehab Seminars reserves the right to cancel the course if minimum enrollment is not met. Registrants for the course will be notified via email with written notification. Registration fees will be refunded in full. However, Advanced Rehab Seminars cannot accept responsibility for other costs incurred by the registrant such as airline or hotel fees and/or penalties.
Cancellation of an In-house course needs to be done a minimum of 45 days in advance. If an In-house course is cancelled after the 45 day mark, the hosting facility will reimburse both speakers travel costs, and 20% of the agreed course fees for canceling at the last minute.
Course sessions and food functions are for course participants only. We regret that spouses and families are not invited to these functions.
